Money Follows the Person
Quality Trust has been partnering with advocacy organizations and government agencies to bring the "Money Follows the Person (MFP) " demonstration grant to the District of Columbia. MFP is an opportunity for states to "rebalance" their long term care systems. The District will take advantage of enhanced Medicaid funding to assist 10 people with developmental disabilities to transition from institutional settings to live in a least restrictive environment in the community. Securing housing is a large part of the process, but more important is developing the wrap around services that each person will need to ensure that the transition is successful. Transportation, direct supports, access to high quality health care, and creating or maintaining personal relationships are also vital requirements for those choosing to participate in the initiative.
